Fenway Health is a federally qualified health center that provides care to a diverse population with a focus on LGBTQIA+ populations. ICH worked with Fenway Health to support the development, implementation, and evaluation of an electronic tablet-based patient-reported outcomes screening system that included social determinants of health-related domains (such as violence and behavioral health).
ICH facilitated discussions that identified how the program would achieve its intended outcomes and which key processes needed to take place to ensure impact. These discussions centered on understanding the workflows and indicators of whether the workflows were working – this in turn helped Fenway refine their processes. ICH then helped Fenway articulate a framework for the program, as well as metrics that would allow Fenway to understand and refine program development. In addition, ICH worked with Fenway to examine how program data would be tracked and how that information would feed into ongoing processes to improve workflows. Finally, ICH designed and helped analyze survey data from stakeholders to better understand opportunities for improvement.
Overall, ICH’s TA helped Fenway Health develop and refine a screening process that improved their ability to identify and address the needs of their diverse and complex patient population.
