RIZE Linkages to Care (LinC) evaluation

ICH is engaged with the RIZE Massachusetts Foundation to provide evaluation support for the Linkages to Care (LinC) Program. This initiative awarded grants to eight community based organizations (CBOs) to improve linkages to substance use disorder (SUD) care following hospitalization by strengthening their partnerships with healthcare centers. This initiative was designed based on findings from a previous ICH report that highlighted the critical importance of smooth connections to care after hospital discharge. In this evaluation, ICH is utilizing a participatory approach to work closely with the CBOs on the collaborative creation of process maps of hospital linkages workflows, identification of evaluation metrics, and development of reporting templates. Throughout the process, ICH is providing technical assistance to support sites in documenting their work and building their evaluation capacity. This data tracking and progress reporting will be complemented by qualitative data collection with staff and clients/patients to understand successes, challenges, process adaptations, and impacts of improved linkage to care. Overall, the evaluation aims to describe the implementation and outcomes of each LinC program as well as identify broader lessons for the field about how to build successful linkages between inpatient care and community-based SUD services.
