The Cambridge Health Alliance (CHA) Department of Medicine and the Institute for Community Health, in conjunction with the Department of Population Medicine, a Harvard Medical School-affiliated research group, were recently awarded a $1.4 million RO1 grant from the Agency for Healthcare Research and Quality (AHRQ). The study has two main objectives: (1) to evaluate CHA’s work to improve care coordination after hospital discharge, and (2) to identify new strategies to lower hospital readmission rates.

The Site PI and CHA partner is Dr. Richard Balaban, MD. ICH Staff involved with this initiative include ICH Director of Evaluation Elisa Friedman, MS and ICH Research Associates Brianna Mills, MA and Jae Goines, BS. Congratulations to the project team!
