The Cambridge Health Alliance (CHA) Department of Medicine and the Institute for Community Health, in conjunction with the Department of Population Medicine, a Harvard Medical School-affiliated research group, were recently awarded a $1.4 million RO1 grant from the Agency for Healthcare Research and Quality (AHRQ). The study has two main objectives: (1) to evaluate CHA’s work to improve care coordination after hospital discharge, and (2) to identify new strategies to lower hospital readmission rates.
