ICH’s Dr. Leah Zallman and Lynn Ibekwe partnered with the Cambridge Health Alliance (CHA) and The Department of Population Medicine (DPM) at Harvard Pilgrim HealthCare Institute to develop a Community Wellness Map – an interactive, web-based tool to help residents be active, eat well, and live healthy. The map provides wellness resources for five towns in CHA’s catchment area: Cambridge, Somerville, Malden, Everett and Revere. It features a comprehensive list of physical activity, nutrition, and weight management programs, and includes key program aspects, such as cost, contact information, and eligibility criteria.
The map was envisioned as formative work informing a larger effort to improve diabetes prevention for CHA patients and individuals living in the surrounding communities. In addition to CHA and DPM, the map was born out of partnership with The Harvard Center for Geographical Analysis and the Dana Farber Cancer Institute Health Communication Core. This project demonstrates the feasibility of creating mapped community program guides, and exemplifies CHA and ICH’s commitment to leveraging partnerships for community health improvement
