ICH served as the evaluator of MelroseWakefield Hospital’s (MelroseWakefield) Determination of Need (DON) Community-Based Health Initiative (CHI) grant-making program, which ended in March 2025. Under the oversight of their Community Benefits Advisory Council, made up of key interest holders from the towns in MelroseWakefield’s service area, the hospital awarded ten grantees over four years to work on initiatives focused on improving behavioral health-related outcomes. As the evaluator, ICH contributed to the process of creating the RFP, and designed a project logic model and evaluation plan. Over the grant period, ICH provided individual evaluation technical assistance to grantees and developed annual evaluation reports based on grantee reporting. In year 4 ICH conducted interviews with each of the grantees and developed an end of grant report that examined learnings from the four years and funding impacts.
