Evaluation of the MetroWest Care Connection

ICH is working with the MetroWest Health Foundation to conduct a process and early outcomes evaluation of the MetroWest Care Connection—a health and human services hub model being implemented in Framingham, Natick, and Ashland to coordinate care and eliminate barriers to the social determinants of health for people with complex care needs. ICH will triangulate findings from an analysis of existing Care Connection program data with insights gained from focus groups and interviews with key informants, staff, and community members in order to assess challenges and build on the successes of the three distinct but overlapping activities of the Care Connection Initiative so far: service navigation, referral management, and complex care coordination. This mixed methods evaluation will allow the MetroWest Health Foundation to make data-informed decisions about how to maximize the program’s impact and expand its reach in the future.
