ICH has worked with MelroseWakefield Hospital (formerly Hallmark Health System) to conduct their Community Health Need Assessments and Community Health Improvement Plans since 2012.
ICH conducted three community health needs assessments and prepared two community health improvement plans for MelroseWakefield Healthcare.
