ICH has worked with MelroseWakefield Hospital (formerly Hallmark Health System) to conduct their Community Health Need Assessments and Community Health Improvement Plans since 2012.
ICH created a new prioritization and assessment tool for Cambridge Homeless Services to use when determining re-housing opportunities for clients.
ICH utilized electronic health record data to create quantitative descriptions of Cambridge Health Alliance patients with specific health conditions.
